The short answer
Some side effects wait. Hair shedding usually starts a few months after weight loss begins, gallstones are tied to substantial or rapid weight loss, and nausea or new skin sensations can arrive with a late dose step, such as Wegovy 7.2 mg. Most are manageable. Severe belly pain, yellow skin or eyes, vision changes with diabetes, or a racing heart at rest need a call the same day.
At GLP-1 SOS the month-four and month-five messages sound alike: “I was fine for months, so why now?” The first weeks get all the attention, but the labels and the research show that a few effects belong to later months. That is not the medication suddenly turning on you. It is usually the weight you have lost, or a dose you have only just reached.
At a glance
Why a side effect can wait months
1
The weight loss itself
Hair shedding and gallstones follow the weight that has come off, so they tend to appear months in.
2
A late dose step
Zepbound and Wegovy climb for months. A new top dose can bring nausea, or with Wegovy 7.2 mg, skin sensations.
3
Rare, any-time problems
Pancreatitis, kidney injury from dehydration and eye changes in diabetes are not tied to week one.
What is actually happening
Hair runs on a delay. The American Academy of Dermatology lists losing 20 pounds or more as a common trigger of excessive shedding, and says most people notice it a few months after the trigger. On Zepbound, hair loss was reported by 7.1 percent of women against 1.3 percent of women on placebo, and the label ties it to weight reduction. More on the timeline in how long GLP-1 hair loss lasts.
Gallstones follow the same logic. The Wegovy label states that substantial or rapid weight loss can raise the risk, and the trials found gallstones in 1.6 percent of adults on the injection against 0.7 percent on placebo. The label adds that the risk stayed higher even after accounting for how much weight people lost. The signs to know are in gallbladder symptoms on a GLP-1.
Lab note
Early side effects follow the dose. Late ones often follow the weight.
Then there are the late dose steps. Zepbound rises in 2.5 mg steps at least 4 weeks apart, and Wegovy can now go from 2.4 mg to 7.2 mg after at least 4 weeks on 2.4 mg. At 7.2 mg, 22 percent of adults reported dysesthesia, meaning tingling, burning, pain or extra sensitivity of the skin, against 6 percent on 2.4 mg. Nausea also returns at a new dose for many people, which we cover in why nausea comes back months in.
From the labels
Three numbers worth knowing
7.1 in 100
women on Zepbound who reported hair loss in the trials, against 1.3 in 100 women on placebo and 0.5 in 100 men on Zepbound
1.6 in 100
adults on Wegovy injection with gallstones in the weight trials, against 0.7 in 100 on placebo, per the Wegovy label
22 in 100
adults on Wegovy 7.2 mg who reported skin sensation changes, against 6 in 100 on 2.4 mg, per the Wegovy label

What to do
- Match the symptom to the calendar. Write down when you started, every dose step, and when the new symptom began. Shedding at month four after steady loss and tingling two weeks after a new dose point to different causes.
- Give hair the protein and the patience. Shedding from weight loss usually settles once your weight steadies. Eat enough protein, and ask your doctor about checking iron and vitamin D. If you want daily support, GLP-1 SOS Daily Hair Shield brings 18 mg of iron, 15 mg of zinc, vitamin D3 and biotin in 2 capsules with dinner, designed to support healthy hair during weight loss. Tell your doctor about the biotin before any blood test.*
- Know the gallbladder pattern. Pain in the upper right belly that lasts hours, with nausea, fever or yellowing of the skin or eyes, needs a same-day call. Do not wait for a second attack.
- Treat a late dose step like a first one. Small, low-fat meals in the first weeks at a new top dose. If skin tingling, burning or nausea is hard to live with, ask about staying on the lower dose; the Wegovy label lets prescribers consider tolerability when choosing the maintenance dose.
- Keep fluids steady through every bad week. The Wegovy and Zepbound labels warn that dehydration from vomiting or diarrhea has led to kidney injury. A long queasy stretch months in deserves the same attention as one in month one.
- With diabetes, keep your eye exams. The Ozempic and Mounjaro labels note that fast improvement in blood sugar has been linked to temporary worsening of diabetic retinopathy. Report any change in vision.

Two kinds
A late but expected change, or a same-day call
Usually expected
- Hair shedding a few months after weight loss starts
- Nausea for a few weeks after a new top dose
- Mild tingling or sensitive skin after a step up
- Constipation or loose stools returning at a new dose
Call the same day
- Pain under the right ribs lasting hours, or fever
- Yellow skin or eyes, or tea-colored urine
- Severe belly pain that spreads to your back
- Racing heart at rest lasting several minutes
A late side effect is not a sign the medicine has stopped working. It is a reason to look at the dose and the pace.
When to call your doctor
Call your prescriber the same day if:
- Pain in your upper right belly lasts several hours, or comes with fever or chills
- Your skin or the whites of your eyes turn yellow, or your urine looks like tea
- Severe belly pain does not go away and may spread to your back, with or without vomiting
- Your heart races or pounds at rest for several minutes
- You have diabetes and notice any change in your vision
- You cannot keep fluids down or feel faint when you stand
- Your mood drops for weeks; if you have thoughts of harming yourself, call or text 988 (Suicide & Crisis Lifeline)
Months in, a new symptom deserves the same care as a first-week one. Most are manageable; a few are urgent.

Questions people also ask
Can Ozempic or Wegovy side effects start after months?
Yes. Some are timed by weight loss rather than the first dose: hair shedding usually shows a few months after weight loss begins, and gallstones are linked to substantial or rapid weight loss. Others arrive with a later dose step, including Wegovy 7.2 mg, where 22 percent of adults reported skin sensation changes. Severe belly pain or yellowing needs a same-day call.
Why am I losing hair 4 months into Zepbound?
That timing fits shedding triggered by weight loss. The American Academy of Dermatology lists losing 20 pounds or more as a trigger and says shedding shows a few months later. The Zepbound label ties hair loss to weight reduction: 7.1 percent of women reported it, against 1.3 percent on placebo. It usually settles as weight steadies; ask your doctor about iron and vitamin D.
Can a GLP-1 cause gallbladder problems months later?
Yes. The labels list acute gallbladder disease, and the Wegovy label says substantial or rapid weight loss can raise the risk, which builds over months. In the Wegovy trials, 1.6 percent of adults on the injection had gallstones against 0.7 percent on placebo. Pain under the right ribs lasting hours, fever or yellow skin means a same-day call.
What is the tingling or burning skin on Wegovy 7.2 mg?
The label calls it dysesthesia: tingling, burning, pain or extra sensitivity of the skin. It was reported by 22 percent of adults on 7.2 mg, against 6 percent on 2.4 mg, and rose with dose and drug levels. In the trials it resolved faster when the dose was reduced or paused. Tell your prescriber rather than pushing through. See skin changes on a GLP-1.
Do side effects get worse the longer you take a GLP-1?
Mostly no. The labels say most nausea, vomiting and diarrhea happen during dose escalation and ease over time. What changes later is the type: effects tied to weight loss, like hair shedding and gallstones, and effects tied to a new higher dose. If a symptom keeps growing on a steady dose, that is worth a call.
