The short answer
Skin changes on a GLP-1 fall into four groups. Dry, itchy skin usually follows less fluid and food. Skin that burns or hurts to touch is listed on the Wegovy label as dysesthesia and rises with dose. A rash or hives soon after a dose can be an allergic reaction and needs a call. Acne is not on the labels. Swelling of the face, lips or throat, or trouble breathing, is an emergency.
Skin questions reach GLP-1 SOS in many forms: a new itch, skin that hurts under clothes, a rash after a dose, breakouts at fifty. They have different causes and different answers. This page sorts them, says what the prescribing information lists, and draws a clear line around the rashes that cannot wait.
At a glance
Three kinds of skin change
1
Dry and itchy
Most often a fluid story: less thirst, smaller meals, water lost through the gut. Gentle care and steady sipping help.
2
Sore or oversensitive
Burning, tingling or skin that hurts to touch. Listed on the Wegovy label, more common at higher doses. Tell your prescriber.
3
Rash or hives
Can be an allergic reaction, especially soon after a dose. Photograph it and call. Swelling or trouble breathing means 911.
What is behind each one
Sore, burning or oversensitive skin has a name on the Wegovy label: dysesthesia, meaning altered skin sensations such as tingling, skin pain and burning. It rose with dose and drug levels: in the 7.2 mg injection trial, 22 percent on that dose, 6 percent on 2.4 mg and 0.3 percent on placebo; on the 25 mg tablet, 5 percent against none. On Zepbound it was under half a percent. The label does not explain why, but reports that lowering or pausing the dose helped episodes resolve faster.
Lab note
Skin that hurts to touch is not in your head. It is on the label, and it is dose-related.
Rashes and itching are a different matter. On Zepbound, hypersensitivity reactions were reported by 5 percent of adults against 3 percent on placebo, and the label says most were skin reactions such as rash and itching. Reactions within a day of a dose were 2.1 percent against 0.4 percent. Severe reactions were rare, about 0.1 percent, but both semaglutide and tirzepatide labels report anaphylaxis and angioedema after approval. Redness or itching right where you inject is more common still: 6 to 8 percent on Zepbound against 2 percent on placebo.
Dry skin is mostly about water. A GLP-1 dulls thirst and shrinks meals, and MedlinePlus lists dry, cool skin among the first signs of mild dehydration. Acne is the honest gap: no label lists it. When breakouts appear, look at what else changed, such as new shakes, new skin products, short sleep or hormones.

of adults on Wegovy 7.2 mg reported altered skin sensations, versus 6% on 2.4 mg, per the Wegovy label
5% vs 3%
hypersensitivity reactions on Zepbound versus placebo, mostly rash and itching, per the Zepbound label
6 to 8%
injection-site reactions on Zepbound, such as redness or itching, versus 2% on placebo

What to do
- Sort it before you treat it. Note where it is, what it feels like (dry, burning, raised, pimples) and which day of your shot week it started. Take a photo for your prescriber.
- Care for dry skin the dermatologist way. Keep showers to five to ten minutes in warm water, use a gentle cleanser, pat dry and apply a fragrance-free cream while the skin is still damp. Run a humidifier at night in dry weather.
- Sip on a schedule. Dry skin and a dry mouth often travel together on a GLP-1. A glass at set times through the day fixes both. Here is how much water to drink on a GLP-1, and what helps a dry mouth on a GLP-1.
- Report skin that burns or hurts to touch. Tell your prescriber, especially after a dose increase. The Wegovy label reports that lowering or pausing the dose helped these episodes resolve faster. Until then, loose cotton clothing rubs less.
- Treat a new rash or hives as a same-day call. Photograph it and call your prescriber before your next dose. Ask your pharmacist whether an antihistamine is safe for you in the meantime.
- Look at what changed for breakouts. New shakes, new creams, short sleep. If a milk-based shake arrived at the same time as the pimples, try a different protein for a few weeks. Persistent or painful acne is worth a dermatologist visit.
- Feed skin and hair from the inside. Keep your daily protein up even when appetite is low. If thinning hair has joined the skin changes, that is the weight-loss shedding we explain in what to do about hair loss on a GLP-1, and the reason we made GLP-1 SOS Daily Hair Shield: iron, zinc, vitamin D3, B12 and biotin in 2 capsules with dinner. Tell your doctor you take biotin before any blood test.*

Questions people also ask
Can Ozempic or Wegovy cause itchy skin?
It can. Itching can come from dry skin, from a reaction at the injection spot, or from an allergic reaction to the medication. Dry skin improves with steady fluids and fragrance-free moisturizer. Itching that spreads, comes with hives, or starts within a day of a dose needs a call before your next dose, and swelling of the face or throat needs 911.
Why does my skin hurt to touch on Wegovy?
The Wegovy label lists dysesthesia: altered skin sensations such as tingling, burning or skin that hurts to touch. It rose with dose, from 6 percent on 2.4 mg to 22 percent on the 7.2 mg injection, and 5 percent on the 25 mg tablet. Lowering or pausing the dose helped it resolve faster, so tell your prescriber rather than pushing through.
Can Mounjaro or Zepbound cause a rash?
Yes. In the Zepbound trials, hypersensitivity reactions were reported by 5 percent of adults against 3 percent on placebo, and most were skin reactions such as rash and itching. Serious reactions, including anaphylaxis and angioedema, are rare but reported. Photograph any new rash, call your prescriber before your next dose, and get emergency help for swelling or trouble breathing.
Does a GLP-1 cause acne?
The labels do not list acne. When breakouts appear, look at what else changed: new protein shakes, new skin products, less sleep or hormonal shifts. The American Academy of Dermatology notes some studies link cow’s milk to breakouts, so a milk-based shake is worth swapping for a few weeks. See a dermatologist if acne is painful or does not settle.
Is dry skin a sign of dehydration on a GLP-1?
It can be one of the first. MedlinePlus lists dry, cool skin among the signs of mild dehydration, along with thirst, dark urine and headache. A GLP-1 dulls thirst and shrinks meals, so fluid intake drops quietly. Drink on a schedule, add a fragrance-free moisturizer, and call your prescriber if you also feel dizzy, confused or are vomiting.
