Is bloating normal on a GLP-1?

The short answer

Yes, especially in the first weeks and after each dose increase. The medication slows how fast your stomach empties, so food and gas sit longer, ferment more, and stretch the belly. Constipation makes it worse. It usually eases within a couple of weeks of each step. Pain, a hard swollen belly or vomiting is not bloating and needs a call.

What is actually happening

A GLP-1 medication slows how fast your stomach empties. The prescribing information for Ozempic, Wegovy, Mounjaro and Zepbound all state it, and the Zepbound label adds that the delay is largest after the first dose and diminishes over time. A stomach that empties slowly is a stomach that stays full. Dinner that used to be gone in a couple of hours is still there at bedtime, pressing outward under the ribs, and that fullness is the first kind of bloating.

The second kind is gas. Food that sits longer in the stomach and small intestine gives bacteria more time to ferment it, especially sugars, sugar alcohols and raw vegetables, and fermentation makes gas. In the Wegovy trials, abdominal distension, burping and flatulence were all reported more often than on placebo, by roughly seven, seven and six adults in a hundred, and the label notes that these reactions were most frequently reported during dosage escalation.

The third kind is swallowed air. Nausea makes people gulp, sip constantly, chew gum and drink carbonated water, and every one of those adds air to a stomach that no longer clears it quickly. Burping was reported by about seven in a hundred adults on Wegovy against fewer than one in a hundred on placebo, and the Zepbound label lists it too.

The hidden cause is constipation. About one adult in four reported it in the Wegovy trials. A colon holding several days of stool takes up room, traps gas behind it, and produces a belly that looks and feels swollen even when the stomach itself is empty. Many people respond by eating less, which slows the colon further. If you have not had a full bowel movement in three days, the bloating is downstream, not upstream.

All of this peaks in the first weeks and after each dose increase, when the slowdown is strongest, and eases as the gut adapts.

What to do

  1. Smaller, slower, earlier. Half the usual portion, chewed until it is soft, and dinner finished three hours before bed so the stomach has time to clear before you lie down.
  2. Cut the gas makers. For a week, skip beans, onions, broccoli, cabbage, raw salads, sugar-free gum and candy with sorbitol or xylitol, and carbonated drinks. Bring them back one at a time.
  3. Stop swallowing air. Sip instead of gulp, skip straws and gum, and eat sitting down without talking on the phone. Air in is gas out, and a slow stomach holds it longer.
  4. Ginger and peppermint after meals. A cup of ginger tea eases the nausea-driven fullness and peppermint relaxes the gut wall; both are old, cheap and reasonable. Can I take Gas-X (simethicone) on a GLP-1?

Simethicone breaks up gas bubbles in the gut and is not absorbed, so it is generally considered low risk, but check with your pharmacist or prescriber first, as with any new product. It will not empty a full stomach or move a stuck colon, which are the two causes it cannot touch. If bloating comes with pain, vomiting or a hard belly, that is a call, not a chewable.
