How to restart a GLP-1 after a break

The short answer

Do not simply pick up at your old dose. After a break, your gut is no longer used to the medication, and the labels build doses up slowly to limit nausea, vomiting and diarrhea. The Wegovy label says that if 2 or more weekly doses in a row are missed, the dose build-up should restart at a lower dose; the Foundayo label says the same after 7 or more daily doses, and Saxenda after more than 3 days. The Ozempic, Zepbound and Mounjaro labels give no rule for long breaks, so your prescriber chooses the restart dose.

At GLP-1 SOS we hear from people coming back after a shortage, an insurance gap, an illness, surgery or a few months off. The question is always the same: can I just take my old dose? This page covers what each label says and how to make the restart week easier. For the weeks after stopping itself, see what happens when you stop a GLP-1.

At a glance

Before your first dose back

1

Count the gap

How many doses did you miss, and when was the last one? That number decides which label rule applies.

2

Call before you inject

Restarting at your old dose after weeks off is what the build-up exists to avoid. Your prescriber sets the new start.

3

Expect a few rough weeks

Side effects cluster during dose escalation, so a restart can feel like month one again.

Why a break changes the dose you can handle

Every GLP-1 starts low and steps up, usually every 4 weeks, and the labels say why: to reduce the risk of stomach and gut side effects. The Wegovy label notes these were reported most often during dose escalation, and that severe gut reactions occurred in 4.1% of people on the injection versus 0.9% on placebo. The Zepbound label adds that the slowing of the stomach is largest after the first dose and eases over time.

A break undoes some of that adjustment. Semaglutide stays in the body for about 5 to 7 weeks after the last dose and tirzepatide has a half-life of about 5 days, as explained in how long a GLP-1 stays in your system. Once it has cleared, your old maintenance dose lands on a gut that has forgotten it, much like jumping straight to 2.4 mg in week one.

Lab note

The dose you ended on is not the dose you restart on. The gap decides.

Here is what each label says. Wegovy injection: if 2 or more consecutive doses are missed, restart the build-up at a lower dose, and its medication guide tells patients to call their provider about how to restart. Foundayo: after 7 or more consecutive missed doses, restart the build-up lower. Saxenda: if more than 3 days have passed, restart at 0.6 mg daily. Ozempic, Zepbound and Mounjaro only cover a single missed dose, not long breaks, and the Wegovy tablets and Rybelsus labels only say to skip a missed dose. Where a label is silent, your prescriber decides.

From the labels

When the labels say to restart lower

2 or more

consecutive missed weekly doses of Wegovy injection: restart the dose build-up at a lower dose (Wegovy label)

7 or more

consecutive missed daily doses of Foundayo (orforglipron): restart the build-up at a lower dose (Foundayo label)

More than 3 days

since the last daily Saxenda dose: restart at 0.6 mg and follow the build-up again (Saxenda label)

How to restart well

  1. Write down the gap before you call. The date of your last dose, how many doses you missed, your old dose and why you stopped. Your prescriber needs all four to pick a restart dose.
  2. Let your prescriber choose the restart dose. It may be the starting dose or a step below your old one; the Wegovy label says only “a lower dosage”. Do not use leftover higher-dose pens without asking.
  3. Get the right pen or prescription first. A lower restart dose often means a different pen strength or a new prescription. Sort it out before injection day so you are not tempted to use what is in the fridge.
  4. Plan the first two weeks like month one. Small meals, early fluids, and the habits in what is normal after a dose increase. Expect the first few shots to be the hardest.
  5. Support your gut from day one. GLP-1 SOS Daily Gut Relief, 2 capsules with dinner, supports regular bowel movements and digestive comfort with magnesium, ginger, peppermint and probiotics while your gut readjusts.*
  6. If you have diabetes, recheck your other medicines. If insulin or a sulfonylurea was raised during the break, restarting a GLP-1 can push blood sugar lower. The labels say those doses may need to come down; check readings more often.
  7. If the break was for surgery, follow the surgical team first. Restart when you are eating normally and your team agrees; see stopping a GLP-1 before surgery.

The restart weeks

Expected, or a same-day call

Usually expected

  • Nausea or early fullness in the first weeks
  • Smaller appetite returning quickly
  • Constipation or loose stools as your gut adjusts
  • Tiredness on the days after a shot

Call the same day

  • Vomiting and unable to keep liquids down
  • Severe belly pain that spreads to your back
  • Very little urine, dark urine or dizziness when standing
  • Swelling of the face, lips or throat, or trouble breathing

A restart that feels like month one is normal. One that feels like an emergency is not.

When to call your doctor

Call your prescriber the same day if, after restarting:

  • You are vomiting and cannot keep liquids down
  • You have severe belly pain that will not go away, with or without vomiting, which can be a sign of pancreatitis
  • You have pain in the upper right belly, fever, or yellowing of your skin or eyes, possible gallbladder problems
  • You have signs of dehydration: very little urine, dark urine, dizziness
  • You take insulin or a sulfonylurea and feel shaky, sweaty or confused

Swelling of the face, lips or throat, or trouble breathing, is an emergency. Call 911.

Questions people also ask

Can I restart Wegovy at the same dose after missing 2 weeks?

Not according to the label. Wegovy’s prescribing information says that if 2 or more consecutive doses are missed, the dose build-up should restart at a lower dose to reduce the risk of stomach and gut side effects, and the medication guide tells you to call your provider about how to restart. Your prescriber decides how low to go.

How do I restart Zepbound or Mounjaro after a break?

Call your prescriber first. The Zepbound and Mounjaro labels cover a single missed dose (take it within 4 days, otherwise skip it) but give no rule for longer breaks. Because the build-up exists to protect your gut, your prescriber may restart you lower after several weeks off. Do not inject an old higher-dose pen without asking.

I missed one Ozempic dose. Do I need to restart?

No. One missed dose is not a break. The Ozempic label says to take it as soon as possible within 5 days; if more than 5 days have passed, skip it and take the next dose on your usual day. Restarting the build-up matters after longer gaps, so if you have missed several weeks, call your prescriber.

How do I restart the Wegovy pill or Rybelsus after stopping?

Both labels say only to skip a missed daily dose and take the next one as planned; neither sets a rule for longer breaks. The Foundayo label, by contrast, says to restart the build-up lower after 7 or more missed daily doses. After a long gap on any pill, ask your prescriber which dose to restart on.

Will I get the same side effects as when I first started?

You might. The labels say stomach and gut side effects happen most during dose escalation, and a restart is a new escalation. The upside is that you now know what helps: small meals, early fluids and a plan for constipation. If symptoms are severe, call your prescriber rather than pushing through.
