Is heartburn normal on a GLP-1?

The short answer

Yes, it is common. A GLP-1 keeps food in your stomach longer, so there is more to push back up, especially after a large or fatty meal or when you lie down. Indigestion was reported by 9 in 100 adults on Wegovy versus 3 on placebo. Smaller, earlier, lower-fat dinners, sleeping with your head raised and skipping your trigger foods help most. Chest pain, trouble swallowing or vomiting blood needs urgent care.

At GLP-1 SOS heartburn messages usually come late in the evening, from people who never had reflux before the medication. The burn is real, and so is the reason: the same slowdown that helps you feel full keeps dinner in your stomach well into the night.

At a glance

Why it burns more on a GLP-1

1

Food sits longer

A slower stomach stays fuller for hours, and a full stomach pushes on the valve at its top.

2

More burps

Burping is that valve opening. Each one can let a little acid up with the air.

3

Late, big, fatty meals

Fat slows the stomach further. Lie down on a full stomach and gravity stops helping.

What is actually happening

Every GLP-1 label says the medication delays gastric emptying. A meal that used to leave your stomach in a couple of hours now stays longer, and the stomach stays stretched. At the top of the stomach a ring of muscle, the lower esophageal sphincter, keeps acid where it belongs. Pressure from below and the repeated opening that comes with burping both give acid more chances to rise. Our page on gas and burping on a GLP-1 covers the burp side.

Lab note

The medication keeps dinner in your stomach longer. Heartburn is what happens when you lie down on it.

The labels count it. In the Wegovy weight-loss trials, indigestion was reported by 9 in 100 adults versus 3 on placebo, reflux disease by 5 versus 3, and burping by 7 versus fewer than 1. On Zepbound, reflux disease ran from 4 to 5 in 100 against 2, and the oral GLP-1 Foundayo lists it in 6 to 7 in 100 against 2. Gut reactions were reported most often during dose escalation.

Two things work in your favor. Gut reactions ease after escalation for most people, and NIDDK lists losing excess weight among the changes that help reflux. Until then, timing and portion size do most of the work, and a plan for a full, bloated stomach often settles the burn too.

What to do

  1. Make dinner the smallest meal, and the earliest. Half the portion, finished at least three hours before bed. A slow stomach needs that time to clear before you lie down.
  2. Take the fat down at night. Fried food, creamy sauces and fatty meat slow the stomach further. Save richer meals for midday, and keep dinner lean and plain.
  3. Test the usual triggers one at a time. NIDDK lists coffee and other caffeine, alcohol, chocolate, mint, spicy food, citrus, tomatoes and high-fat food. Drop them for a week, then bring back one every few days to find yours. Our page on coffee on a GLP-1 has gentler ways to keep it.
  4. Raise your head, not just your pillow. A foam wedge or a raised head of the bed that lifts your head and upper back 6 to 8 inches, as NIDDK suggests. The goal is an incline from the waist up, not a bent neck.
  5. Stay upright after eating. An hour sitting or walking after meals: no couch, no bending to clean right after dinner, and loose waistbands in the evening.
  6. Use the pharmacy shelf wisely. Antacids for an occasional bad night; ask your pharmacist about an H2 blocker or a proton pump inhibitor if it happens most days. Long-term acid reducers can lower B12 absorption, which is one reason to read should I take B12 on a GLP-1. If you take the Wegovy pill or Rybelsus, keep antacids away from the tablet’s 30-minute window.
  7. Tell your prescriber if it lasts. Heartburn most days for more than two weeks, or any of the warning signs below, is worth a call. A slower dose step is a normal thing to ask for.
GLP-1 SOS support team

From our support desk at GLP-1 SOS

If you wrote to us tonight about heartburn, here is what we would say.

Tonight: nothing more to eat, prop your head and shoulders on a wedge, sip water, and take an antacid if your pharmacist has said it is fine for you. Tomorrow: a smaller dinner three hours before bed, and a week without coffee, alcohol, chocolate, mint and fried food to find your trigger. We would not offer anything of ours as a heartburn fix. Where GLP-1 SOS Daily Gut Relief helps is the gut picture that often comes with it: 200 mg of ginger for queasy, overfull evenings and 250 mg of magnesium for slow bowels, in 2 capsules with dinner; ginger tea does part of the same job.*

We answer real questions from real people every day. Write to us if yours is not here.

Two kinds

GLP-1 heartburn, or a same-day call

Usually GLP-1 heartburn

  • Burning behind the breastbone after meals
  • Worse lying down or bending over
  • Sour taste or burps after dinner
  • Worse on the days after your shot
  • Eases with an antacid or sitting up

Get help the same day

  • Chest pain or pressure, especially with breathlessness or sweating
  • Trouble or pain swallowing
  • Vomiting blood, or black, tarry stools
  • Severe upper belly pain spreading to your back
  • Persistent vomiting

When in doubt about chest pain, call 911. Heartburn can wait; a heart cannot.

When to call your doctor

Call 911 for chest pain or pressure that could be your heart. Call your prescriber the same day if:

  • You have trouble swallowing, pain when swallowing, or food feels stuck
  • You vomit blood or material that looks like coffee grounds, or your stools are black and tarry
  • You have severe upper belly pain, especially spreading to your back
  • You have pain under your right ribs, fever, or yellow skin or eyes
  • You keep vomiting, or heartburn wakes you most nights despite treatment

Burning after a big dinner is the slow stomach. Pain with swallowing, blood or chest pressure is not.

Questions people also ask

Does Ozempic or Wegovy cause heartburn?

It can. Both slow how fast the stomach empties, and in the Wegovy weight-loss trials indigestion was reported by 9 in 100 adults versus 3 on placebo, and reflux disease by 5 versus 3. It is usually worst after large, fatty or late meals and in the weeks after a dose increase. Smaller, earlier dinners and sleeping with your head raised help most people.

Why is my acid reflux worse at night on Mounjaro or Zepbound?

Because dinner is still in your stomach when you lie down. Tirzepatide delays stomach emptying, and lying flat removes the gravity that keeps acid down. The Zepbound label lists dyspepsia in 9 to 10 in 100 adults and reflux disease in 4 to 5. Finish dinner three hours before bed and raise your head and upper back 6 to 8 inches, as NIDDK suggests.

Can I take Tums, Pepcid or omeprazole with a GLP-1?

Usually, but ask your pharmacist first. The GLP-1 labels do not list antacids or acid reducers as interactions, and the Wegovy label found no meaningful effect of omeprazole on semaglutide. With the Wegovy pill or Rybelsus, keep everything away from the tablet’s 30-minute window. Years of acid reducers can lower B12, so mention them at your next check.

Does heartburn on a GLP-1 go away?

For many people it eases. The labels report gut reactions most often during dose escalation, and losing excess weight is one of the lifestyle changes NIDDK lists for reflux. What keeps it going is late, large or fatty dinners. If you still have heartburn most days after a few weeks on a steady dose, or it wakes you at night, talk to your prescriber.

How do I know if it is heartburn or something serious?

Heartburn is a burning behind the breastbone after eating that is worse lying down and eases with an antacid or sitting up. Chest pressure or pain with breathlessness, sweating, or pain spreading to your arm or jaw could be your heart: call 911. Trouble swallowing, vomiting blood or black stools also need same-day care, per NIDDK.
