Why does a GLP-1 cause dry mouth (and what helps)?

The short answer

Mostly because you are drinking less. Thirst fades with appetite, meals shrink so your mouth is asked to make saliva less often, and vomiting or loose stools take fluid out. Mouth breathing at night adds to it. Only the Zepbound label lists it, at about one in a hundred. Sipping, salt, sugar-free gum and a humidifier fix most of it.

What is actually happening

Saliva is mostly water, made by glands that switch on when you chew, taste and smell food. Between meals they run at a low resting flow. A GLP-1 medication lowers appetite, so most people eat fewer and smaller meals, and the glands are fully switched on less often. The mouth spends more hours at resting flow, and resting flow feels dry.

The larger cause is fluid. Thirst and appetite drop together, and people who drank eight cups a day find they are at two or three without noticing. Food normally supplies part of your water, through fruit, vegetables and soups, and when meals shrink that share goes with them. Nausea, vomiting and diarrhea, common in the first weeks and after dose increases, take more out; the Wegovy label warns that dehydration from these reactions has strained the kidneys in some patients. The National Library of Medicine lists dry mouth among the first signs of dehydration, with thirst, dark urine, tiredness and dizziness.

Two smaller causes stack up. Mouth breathing at night, common with a stuffy nose or snoring, dries the mouth and throat by morning. And other medicines: the National Institute of Dental and Craniofacial Research notes that hundreds of medications reduce saliva, including common ones for blood pressure, depression and bladder control. Caffeine and alcohol dry it further.

The labels fit this picture. The Zepbound label lists dry mouth or dry throat in about one patient in a hundred, against one in a thousand on placebo. The Wegovy, Ozempic, Mounjaro and Rybelsus labels do not list it at all, which is what you would expect from a symptom driven by how much you drink and how often you eat rather than by the medication acting on the glands.

It matters. Saliva washes the teeth, neutralizes acid and keeps yeast in check, so a dry mouth means more decay and more fungal infections, and dry food is harder to swallow.

What to do

  1. Count your cups for two days. Write down every drink. Most people who ask us about dry mouth are under four cups. The NIDCR dry mouth guidance suggests eight to twelve cups of water a day; if you have a heart or kidney condition, ask your prescriber for your number.
  2. Sip on a schedule, not on thirst. A bottle within reach and a few swallows every twenty to thirty minutes, between meals rather than with them, so a slow stomach is never loaded with food and a full glass at once.
  3. Put salt in some of it. Once a day, a cup of broth, a pinch of salt in water, or a sugar-free electrolyte mix, and more on any day with vomiting or loose stools. Water follows salt, and salt is what small meals stop supplying.
  4. Chew sugar-free gum or suck sugar-free candy. Chewing and taste switch the glands on. Keep it to a few pieces a day, because the sugar alcohols in most of them loosen the stool on a slow gut.
  5. Cut the drying drinks. Swap the second coffee for water, skip alcohol on peak days, and stop tobacco if you use it. All three dry the mouth, and the NIDCR lists each one.
  6. Fix the nights. A humidifier in the bedroom, a glass of water at the bedside, saline spray if your nose is blocked, and lip balm. Dry throat on waking is mouth breathing, and it is the easiest part to solve.
  7. Tell your dentist and keep the checkup. Less saliva means more decay, so use a fluoride toothpaste, rinse with water after any vomiting rather than brushing straight away, and let the hygienist know you are on a GLP-1. If dry mouth stays after two weeks of drinking more, bring your full medication list to your prescriber.

When to call your doctor

Call your prescriber the same day if:

  • You feel faint or dizzy when you stand
  • Your urine is dark or you are passing very little
  • You cannot keep liquids down
  • You have vomited more than once or had diarrhea for more than two days
  • Your heart is racing or you feel confused
  • Your mouth is so dry you cannot swallow food

Also mention it at your next visit if dry mouth has lasted more than two weeks despite drinking more, or if you have white patches in your mouth or a burning tongue. Dry after a small-meal day is water. Dry with dizziness and dark urine is not.

Does Ozempic cause dry mouth?

Not directly, and the Ozempic label does not list it. What causes it is the pattern the medication creates: less thirst, smaller and fewer meals that switch on saliva less often, and fluid lost to nausea, vomiting or loose stools in the first weeks. Of the injectable labels, only Zepbound lists dry mouth or dry throat, at about one patient in a hundred. Drinking more on a schedule usually solves it within days.

Is dry mouth a sign of dehydration on Wegovy or Zepbound?

Often, yes. The National Library of Medicine lists dry mouth among the first signs of dehydration in adults, with thirst, dark urine, tiredness and dizziness. On a GLP-1 the usual cause is simply drinking less because thirst dropped with appetite, plus fluid lost to gut reactions. Count your cups for two days. If dry mouth comes with dizziness on standing or very little urine, call your prescriber the same day.

Why do I wake up with a dry mouth and throat on Mounjaro?

Because you are breathing through your mouth at night and going to bed already short of fluid. Saliva flow drops during sleep for everyone, and a stuffy nose or snoring dries the throat by morning. A humidifier, saline spray before bed, a glass of water at the bedside and steady sipping through the evening usually fix the mornings. If the snoring is new or loud, mention it to your prescriber.

Can dry mouth on a GLP-1 damage my teeth?

It can if it lasts. Saliva washes the teeth, neutralizes acid and keeps yeast in check, and the NIDCR lists tooth decay and fungal mouth infections as the main problems of a dry mouth. Vomiting adds stomach acid to the enamel. Use a fluoride toothpaste, rinse with water rather than brushing right after vomiting, keep sugar-free gum in the bag, and tell your dentist you are on a GLP-1 so they can watch for early decay.
