Should I skip my GLP-1 shot if I’m constipated?

The short answer

Not on your own. No GLP-1 label tells you to skip a dose for constipation; changing your schedule is your prescriber’s call, and for ordinary constipation most people keep their dose and treat the constipation. Prescribers can delay a dose increase or choose a lower dose if a dose is not tolerated. But constipation with belly pain, vomiting, a swollen belly or no gas is different: call the same day, before your next shot.

At GLP-1 SOS we get this question most often the night before shot day: it has been four days, I feel heavy, should I just skip this week? Here is what the labels say, why skipping on your own can backfire, and the signs that make it a different question.

At a glance

Three things to know

1

It is your prescriber’s call

The labels give prescribers options when a dose is not tolerated: delay the next step, or settle on a lower dose. Ask before you change anything.

2

Skipping can backfire

Per the Wegovy label, missing two or more doses in a row means restarting at a lower dose and climbing again, side effects included.

3

Some constipation is urgent

The labels list ileus and intestinal obstruction among rare reports. Pain, vomiting or no gas needs a call today, not a skipped shot.

What the labels say

Constipation is one of the most common side effects of every GLP-1; on Wegovy, 24% of adults had it in the weight trials, versus 11% on placebo (here is how common it is on each medication). None of the labels tell patients to hold a dose for it. They give prescribers room to adjust instead: the Wegovy label says to consider delaying a dose increase by four weeks if a dose is not tolerated, and the Zepbound label says to consider a lower maintenance dose.

Lab note

A slow week is a reason to treat the constipation and call. It is rarely a reason to skip quietly.

Skipping has costs the labels spell out. If you miss a dose, each label has a window: Ozempic within 5 days, Mounjaro and Zepbound within 4 days, and Wegovy as long as your next dose is more than 2 days away. If two or more Wegovy doses are missed in a row, the label says to restart at a lower dose to reduce stomach side effects, which means more weeks of climbing back up. In other words, a skipped week can turn into a fresh round of the very side effects you were trying to avoid.

The exception is the rare serious kind. The Wegovy, Ozempic, Mounjaro and Zepbound labels list ileus, intestinal obstruction and severe constipation with fecal impaction among reports collected after approval. If constipation comes with constant belly pain, vomiting, a hard swollen belly or no gas, the question is not whether to skip your shot. It is a same-day call, and your prescriber will decide the next dose.

What to do the night before shot day

  1. Count the days and check the red flags first. If you have pain, vomiting, a swollen belly or no gas, stop here and call your prescriber or urgent care tonight.
  2. If it is the ordinary kind, treat the constipation. Water through the day, a little more soluble fiber, and one osmotic at the label dose: MiraLAX or magnesium citrate, not both.
  3. Send your prescriber a message before shot day. Say how many days, what you have tried and how you feel. Ask directly: keep my dose, delay my next increase, or hold this week? If a step-up is due, ask whether to wait.
  4. Do not skip on your own. If your prescriber says to hold, follow their plan for the next dose. If you miss a dose by accident, use your label’s missed-dose window rather than doubling up.
  5. If the usual fixes are not working, escalate the plan. Here is what to do when nothing works for constipation on a GLP-1, including when to ask about prescription options.

From our support desk at GLP-1 SOS

If you wrote to us tonight about this, here is what we would say.

Please do not skip on your own; message your prescriber tonight, and if there is pain, vomiting or no gas, call instead. Then plan for the next few weeks, because constipation on a GLP-1 tends to return with each step. That is why we built GLP-1 SOS Daily Gut Relief: 250 mg of magnesium (citrate and oxide) that supports regular bowel movements, plus ginger, peppermint and a 19-strain probiotic blend, in 2 capsules with dinner. It is a daily routine, not a rescue, and it already contains magnesium, so do not stack it with magnesium citrate without asking.* The brand-free version: water, soluble fiber and a walk after dinner.

We answer real questions from real people every day. Write to us if yours is not here.

*GLP-1 SOS Daily Gut Relief supports regular bowel movements and digestive comfort. These statements have not been evaluated by the Food and Drug Administration. This product is not intended to diagnose, treat, cure or prevent any disease.

Two kinds

Treat and call, or call today

Treat it and mention it

  • Three or four days without a bowel movement
  • Hard stools, straining, a heavy feeling
  • Bloating that eases when you go
  • Worse in the weeks after a dose increase

Call the same day

  • More than five days without a bowel movement
  • Constant or severe belly pain
  • Vomiting, or not passing gas
  • A hard, swollen belly, or blood in the stool

Left: keep your schedule unless told otherwise. Right: call before your next dose.

When to call your doctor

Call your prescriber the same day, before your next dose, if:

  • You go more than five days without a bowel movement
  • You have constant or severe belly pain, or a fever with belly pain
  • You are vomiting, or cannot keep liquids down
  • You cannot pass gas, or your belly is hard and swollen
  • There is blood in your stool, or black stool

Ordinary constipation: treat it and ask. Constipation with pain, vomiting or no gas: call today.

Questions people also ask

Should I skip my Ozempic dose if I’m constipated?

Not without asking. The Ozempic label does not tell patients to hold a dose for constipation, and constipation affected 3 to 5% of people in its trials. Treat it with water, fiber and one osmotic, and message your prescriber. If you do miss a dose, the label says to take it within 5 days; after that, skip it and resume your usual day.

Can I delay my Mounjaro or Zepbound shot because of constipation?

Ask your prescriber first. The Mounjaro and Zepbound labels allow a missed dose to be taken within 4 days, and the Zepbound label lets prescribers consider a lower maintenance dose if a dose is not tolerated. Delaying on your own just shifts the same dose later in the week. Constipation with pain, vomiting or no gas needs a same-day call.

What happens if I skip two Wegovy shots?

Per the Wegovy label, if two or more doses in a row are missed, your prescriber should restart you at a lower dose and step you up again, to reduce stomach side effects. That means more weeks of dose increases, and constipation often peaks after each increase. That is a strong reason to ask before skipping, rather than skipping and then calling.

Can a GLP-1 cause a bowel obstruction?

Rarely. The labels list ileus, intestinal obstruction and severe constipation with fecal impaction among reports collected after approval; they cannot give a rate from those reports. Warning signs are constant belly pain, vomiting, a hard swollen belly and not passing gas or stool. Those need a same-day call or urgent care, not a home remedy.

Will constipation get better if I stay on the same dose?

Often, yes. Constipation tends to peak in the weeks after a dose increase, and staying on a dose longer is one option prescribers use. Water, soluble fiber and, if needed, magnesium citrate or MiraLAX usually keep it manageable while you stay on schedule.
