Maintenance dose: what it means on a GLP-1

Definition

The maintenance dose is the dose you stay on once titration ends, such as Wegovy 1.7 or 2.4 mg weekly or Zepbound 5, 10 or 15 mg weekly. The highest dose is not the goal for everyone. The labels say to weigh response and tolerability.

Each label names its own maintenance doses. For weight reduction, Wegovy injection is 1.7 mg or 2.4 mg (recommended) once a week, and adults who tolerate 2.4 mg for at least 4 weeks may go to 7.2 mg if more weight loss is needed. Wegovy tablets are 25 mg a day. Zepbound is 5, 10 or 15 mg a week. Ozempic, for type 2 diabetes, is 0.5, 1 or 2 mg a week. Foundayo does not use the word maintenance; the label allows steps to 9, 14.5 or 17.2 mg based on response and tolerability.

Staying on treatment matters for keeping weight off. In a Zepbound withdrawal trial, adults who kept taking it after 36 weeks lost another 5.5% of body weight by week 88, while those switched to placebo regained 14.0%. Side effects usually ease once the dose stops changing, though some people notice nausea or food noise return months in.

Your prescriber may also lower the dose if side effects do not settle. At GLP-1 SOS we see maintenance as the long road, where daily habits, protein and gut comfort matter more than the number on the pen. For how you got there, see dose titration.
