Is it normal to have no appetite at all on a GLP-1?

The short answer

Often, yes. A GLP-1 lowers calorie intake by turning appetite down, and some people feel almost no hunger, especially early on and after a dose increase; in one trial, people on semaglutide 2.4 mg ate 35% less at a free-choice meal than people on placebo. No appetite is expected, but eating almost nothing is not. Keep fluids, protein and small regular meals, and call your prescriber if you cannot keep liquids down.

At GLP-1 SOS we hear this from two kinds of people. Some are thrilled: food has finally gone quiet. Others are uneasy: they realize at 9 p.m. that they have had a coffee and three crackers all day. Both are describing the same effect. Here is why it happens and where the line sits between a quiet appetite and too little food.

At a glance

Three things to know

1

It is the medicine working

The labels say a GLP-1 lowers calorie intake by acting on appetite. Feeling little or no hunger is that effect, not a malfunction.

2

It often comes in waves

Many people notice it most in the first weeks and after a dose increase, when side effects are most frequent per the labels.

3

Your body still needs a floor

No hunger does not mean no needs. Fluids, protein and a few small meals protect your muscle, your gallbladder and your kidneys.

What is actually happening

GLP-1 is a hormone your gut releases after you eat. It tells the brain you are fed. The medicines copy it at higher, steadier levels, and the Wegovy and Zepbound labels say they decrease calorie intake, likely by acting on appetite. In a 20-week trial, people on semaglutide 2.4 mg ate 35% less at an all-you-want lunch than people on placebo, with less hunger, more fullness and fewer cravings.

Lab note

A quiet appetite is the point of the medicine. An empty plate all day is not.

Some people feel the volume turned down; others feel it switched off. On the Mounjaro label, 5 to 11% of people reported decreased appetite as a side effect, against 1% on placebo, and 17% did in a long heart trial. On Rybelsus it was 6 to 9%, against 1%. Those numbers count people for whom it felt like a problem, not everyone whose appetite dropped. If you have the opposite problem, read why some people are still hungry on a GLP-1.

The risk is not the missing hunger. It is what can follow when intake drops too far for too long: too little protein, too little fluid and very fast weight loss. The labels flag the last two directly, through kidney injury linked to dehydration and gallstones, which become more likely with rapid weight loss.

From the labels and trials

The numbers behind no appetite

35%

less eaten at a free-choice lunch on semaglutide 2.4 mg than on placebo, after 20 weeks (Friedrichsen trial)

5 to 11%

of people on Mounjaro reported decreased appetite as a side effect, versus 1% on placebo, per the Mounjaro label

1.6%

of adults on Wegovy injection had gallstones in the weight trials, versus 0.7% on placebo; rapid weight loss raises that risk, per the label

What to do

  1. Eat by the clock, not by hunger. Hunger is no longer a reliable signal, so set times instead: three small meals, or five mini-meals, with a phone reminder if you forget.
  2. Protein first at every meal. Start with the protein so the few bites you manage count: yogurt, eggs, cottage cheese, fish, a shake. Here are the best protein sources and shakes when you can’t eat much.
  3. Drink on a schedule too. Thirst can fade along with appetite, and food normally supplies part of your daily water. Sip through the day; here is how much water to drink on a GLP-1.
  4. Keep dinner, even if small. A modest, protein-first dinner is where GLP-1 SOS Daily Gut Relief fits: 2 capsules with dinner, with ginger and peppermint that support digestive comfort, plus B12, folate, iron, zinc and vitamin D. We built it for the smaller plates of life on a GLP-1.*
  5. Keep your muscles working. The 2025 joint advisory on GLP-1 nutrition names protein and strength training as the priorities for protecting muscle and bone. More in does a GLP-1 cause muscle loss.
  6. Tell your prescriber if it lasts. If you are eating very little week after week, or losing weight faster than planned, say so. The Wegovy label lets prescribers delay a dose step by four weeks when a dose is not tolerated. That is their call, not yours to make alone.

Two kinds

Quiet appetite, or a same-day call

Usually expected

  • Little or no hunger, but you can eat when you decide to
  • Feeling full after a few bites
  • Forgetting meals unless you set a reminder
  • Stronger in the days after a dose increase
  • Some foods suddenly unappealing

Call the same day

  • You cannot keep liquids down, or vomit repeatedly
  • Dizziness, fainting, very dark urine or very little urine
  • Several days of eating almost nothing
  • Severe belly pain, or pain spreading to your back
  • Shaky, sweaty or confused spells if you use insulin or a sulfonylurea

Not hungry is common. Not able to eat or drink is a call.

When to call your doctor

Call your prescriber the same day if:

  • You cannot keep liquids down, or you are vomiting repeatedly
  • You feel dizzy or faint, or your urine is very dark or scant
  • You have eaten almost nothing for several days
  • You have severe belly pain that will not go away, sometimes spreading to your back
  • You have upper right belly pain, fever, or yellow skin or eyes
  • You use insulin or a sulfonylurea and have shaky, sweaty or confused spells

No appetite is the medicine. No food and no fluids is a call.

Questions people also ask

Is it normal to have no appetite on Ozempic or Wegovy?

Yes, for many people. Both are semaglutide, and the Wegovy label says it lowers calorie intake by affecting appetite. In a trial, people on semaglutide 2.4 mg ate 35% less at a free-choice lunch than people on placebo. Feeling no hunger is common, especially early and after a dose step. Eat on a schedule anyway, and call if you cannot keep fluids down.

Is no appetite normal on Mounjaro or Zepbound?

Yes. Decreased appetite is listed on the Mounjaro label: 5%, 10% and 11% of people on 5, 10 and 15 mg reported it, versus 1% on placebo, and 17% did in a long heart trial. Zepbound is the same medicine, tirzepatide, and its label says it decreases calorie intake through appetite. Treat meals as appointments rather than waiting to feel hungry.

How little is too little to eat on a GLP-1?

The labels do not set a minimum, and the right number depends on your size, age and health, so ask your prescriber or a dietitian for yours. Practical warning signs are days of eating almost nothing, struggling to drink, dizziness, or weight falling much faster than planned. A protein-first meal, even a small one, at regular times is a good floor.

Will my appetite come back on a GLP-1?

Often partly. Appetite is usually quietest in the first weeks and right after dose increases, and many people notice some hunger returning on a steady dose. If food noise comes back strongly, that is a separate question. For most people the goal is a calm, manageable appetite rather than none at all, which is worth discussing with your prescriber.

Can eating too little on a GLP-1 cause gallstones?

It can raise the risk. The Wegovy label says substantial or rapid weight loss can increase the risk of gallstones, and gallstones were more common on Wegovy than placebo even after accounting for weight loss: 1.6% versus 0.7% in the adult weight trials. Pain in the upper right belly, fever or yellow skin or eyes needs a call the same day.
