Wegovy pill vs orforglipron (Foundayo) vs Rybelsus: which oral GLP-1?

The short answer

If the goal is weight loss, the choice is between the Wegovy pill (oral semaglutide 25 mg) and Foundayo (orforglipron). Rybelsus is approved for type 2 diabetes, not weight loss. The Wegovy pill must be taken on an empty stomach with a 30-minute wait and reduced body weight by 13.6% at 64 weeks in its trial; Foundayo can be taken with or without food and reduced it by up to 11.1% at 72 weeks. The trials were separate, so the numbers are not a head-to-head comparison.

At GLP-1 SOS we have heard this question almost daily since Foundayo arrived. Our page on whether the pill is worse for nausea or constipation than the shot covers side effects. This one compares the three pills on what decides the fit: approval, routine, trial results and interactions.

At a glance

Three pills, three profiles

1

Wegovy pill

Semaglutide 25 mg for weight and heart risk. Empty stomach on waking, 4 ounces of water at most, 30-minute wait.

2

Foundayo

Orforglipron for weight reduction. Once daily, with or without food.

3

Rybelsus

Semaglutide for type 2 diabetes. Same empty-stomach rule. Not approved for weight loss.

How the three really differ

Approval comes first. The Wegovy label covers its tablets for weight reduction and for lowering heart risk in adults with heart disease and overweight or obesity. The Foundayo label covers weight reduction and long-term maintenance. Rybelsus, and the newer Ozempic tablets in the same label, are approved for blood sugar control and heart risk in type 2 diabetes. That shapes what a prescriber can write and what a plan may cover.

Then the routine. Semaglutide is a peptide the stomach would digest, so both semaglutide tablets carry an absorption enhancer and an empty-stomach rule: on waking, whole, with up to 4 ounces of plain water, nothing else for 30 minutes. Orforglipron is a small molecule, not a peptide, and the Foundayo label says once daily with or without food. Each dose step lasts at least 30 days. The Wegovy pill reaches its 25 mg maintenance dose on day 91; Foundayo starts at 0.8 mg and can go up to 17.2 mg.

Lab note

Same receptor, different rules. Choose the routine you can keep every single day.

On results, each label reports its own trial. Adults on the Wegovy 25 mg tablet lost 13.6% of body weight at 64 weeks, against 2.4% on placebo. Adults without diabetes on Foundayo 17.2 mg lost 11.1% at 72 weeks, against 2.1%; the 5.5 mg and 9 mg doses gave 7.4% and 8.3%. Different people, lengths and methods mean these are not head-to-head numbers. Gut side effects are the familiar ones on all three: Foundayo lists nausea in 26 to 35 in 100, and Rybelsus, at its diabetes doses, 11 to 20 in 100.

Interactions differ too. A slower stomach can change how other pills are absorbed. With the semaglutide tablet, levothyroxine exposure rose 33% in a study, so thyroid levels may need checking. The Foundayo label asks people on birth control pills to switch to a non-oral method, or add a barrier method, for 30 days after starting and after each dose increase. It also caps the dose at 9 mg with strong CYP3A4 inhibitors such as the antibiotic clarithromycin, and says to avoid some, such as ritonavir.

How to choose with your prescriber

  1. Start from the approval, not the price. For weight loss, the options are the Wegovy pill and Foundayo. With type 2 diabetes, Rybelsus may be the one your plan covers for blood sugar.
  2. Be honest about your mornings. The semaglutide tablets need an empty stomach and the 30-minute wait. If that will not happen most days, say so; the routine is in how to take the Wegovy pill or Rybelsus.
  3. Bring your full medication list. Thyroid medicine, birth control pills, antibiotics and anything with a narrow dose range matter here.
  4. Expect the first weeks to be the bumpy ones. Gut side effects cluster around each 30-day step. See Wegovy pill side effects in week 1.
  5. Plan your supplements around the pill. With a semaglutide tablet nothing else goes in for 30 minutes, so vitamins and minerals belong with dinner.
  6. Ask how you would switch if it does not suit you. The Wegovy label explains switching between its tablet and injection. Knowing the fallback makes the first choice easier.

Fit check

Which pill tends to fit which life

The Wegovy pill may fit if

  • Weight loss or heart risk is the goal
  • Your mornings are predictable
  • You can hold coffee and other pills for 30 minutes
  • You may switch to or from the Wegovy injection

Foundayo may fit if

  • Weight loss is the goal and mornings are chaotic
  • You take other pills first thing
  • You can manage the birth control step if it applies
  • You are not on a strong CYP3A4 inhibitor

Rybelsus fits a different question: blood sugar control in type 2 diabetes.

When to call your doctor

Call your prescriber the same day if:

  • You have severe belly pain that does not go away, with or without vomiting
  • You are vomiting or cannot keep liquids down
  • You have upper right belly pain, fever, or yellow skin or eyes
  • You take insulin or a sulfonylurea and have shaky, sweaty or confused spells
  • You notice a lump in your neck, hoarseness or trouble swallowing
  • You could be pregnant

The best pill is the one you can take correctly every day.

Questions people also ask

Which works better for weight loss, the Wegovy pill or Foundayo?

There is no head-to-head trial in the labels. In its 64-week trial, the Wegovy 25 mg tablet lowered body weight by 13.6% on average, against 2.4% on placebo. In a separate 72-week trial, Foundayo 17.2 mg lowered it by 11.1%, against 2.1%. Different people and methods mean you cannot rank them from these numbers. Your routine and other medicines often decide.

Can I take Rybelsus for weight loss?

Rybelsus is approved for blood sugar control and heart risk in adults with type 2 diabetes, not for weight loss. People with diabetes often lose some weight on it, but that is not its label use. For weight reduction, the oral options with that approval are the Wegovy tablet, also semaglutide, and Foundayo.

Do I need an empty stomach for Foundayo (orforglipron)?

No. The Foundayo label says once daily, with or without food, and sets no time of day or water limit. That is the main practical difference from the Wegovy pill and Rybelsus, which need an empty stomach, up to 4 ounces of plain water and a 30-minute wait. A fixed daily time still helps you remember.

Is the Wegovy pill the same as Rybelsus?

Both contain semaglutide with an absorption enhancer and follow the same empty-stomach rule, but they are different products. The Wegovy tablet is 25 mg for weight reduction and heart risk; Rybelsus comes in lower doses for type 2 diabetes. The doses are not interchangeable, so never swap one for the other without your prescriber.

Does Foundayo affect birth control pills?

Its label says it may. Because it slows stomach emptying, the Foundayo label advises people on oral hormonal contraceptives to switch to a non-oral method, or add a barrier method, for 30 days after starting and after each dose increase. It also says to stop Foundayo when pregnancy is recognized.
