Wegovy pill side effects in week 1: what’s normal

The short answer

Mild ones are normal. Week 1 is the lowest dose, 1.5 mg a day, and the level in your blood is still building, so a little nausea after the dose, early fullness, less appetite, burping or a headache are common. Vomiting that will not stop, severe belly pain, or swelling of the face or throat is not something to wait out.

At GLP-1 SOS the first-week questions about the Wegovy tablet arrive in a familiar order: is this queasy feeling normal, why is it worse after the dose, and when does it stop. Here is what the label says about the start. If you are weighing pill against shot, see whether the pill is worse for nausea or constipation.

Week 1 at a glance

What the first seven days usually look like

1

The lowest dose

1.5 mg once a day for days 1 to 30, chosen to reduce stomach side effects.

2

A building level

Semaglutide lasts about a week in the body, so each daily tablet adds to the last.

3

Mornings carry it

The tablet goes into an empty stomach, so queasiness tends to sit in the first hours.

What is actually happening

The Wegovy tablet is semaglutide, the same medicine as the Wegovy injection, made with an absorption enhancer. The label says it is absorbed mainly in the stomach, with the peak level about one hour after the dose. Hence the rules: one tablet on an empty stomach in the morning with up to 4 ounces of plain water, then at least 30 minutes before food, other drinks or other pills.

Everyone starts at 1.5 mg a day for 30 days, then steps to 4 mg, 9 mg and 25 mg every 30 days, to reduce stomach side effects. Semaglutide has a half-life of about a week, and the semaglutide tablet label says a steady level takes 4 to 5 weeks. So week 1 is the lowest exposure you will have.

Lab note

Week 1 is a rehearsal. The real test of a dose comes in its third and fourth week.

What you feel is the shot’s mechanism at a small scale: a slower stomach and a quieter appetite. For the 25 mg tablet, the label says common side effects were similar in type and frequency to the injection, led by nausea, diarrhea, vomiting and constipation, and were most frequent during dose escalation. In the OASIS 4 trial, 74 percent of adults on the tablet reported a stomach side effect at some point, against 42 percent on placebo.

Two things are specific to the pill. The dose lands in an empty stomach, so queasiness often sits in the first hours. And breaking the 30-minute rule does not cause side effects; it lowers how much medicine gets in.

From the labels and the trial

Three numbers for week 1

1.5 mg

the starting dose, once a day for days 1 to 30, set to reduce stomach side effects, per the Wegovy label

4 to 5 weeks

for daily semaglutide tablets to reach a steady level, per the Rybelsus label, so week 1 is the lowest exposure

74 vs 42%

of adults reporting any stomach side effect over the OASIS 4 trial, on the 25 mg tablet versus placebo

What to do in week 1

  1. Take it the same way every day. On waking, empty stomach, one tablet swallowed whole with up to 4 ounces of plain water, then at least 30 minutes before food, coffee or other pills. If you miss a day, the label says skip it and take the next one the following day.
  2. Break the fast small. After the 30 minutes, something light with protein: yogurt, an egg, toast with cottage cheese. See what to eat when you feel nauseous.
  3. Eat small and often. Every three to four hours, stopping at satisfied. Fat and big portions slow the stomach further.
  4. Sip water all day. Appetite and thirst fall together. Steady sips keep headaches and lightheaded moments away.
  5. Set up the evening now. Week 1 is the easy time to build the routine you will want at higher steps: a small early dinner, a short walk, something for the stomach. GLP-1 SOS Daily Gut Relief is 2 capsules with dinner, far from the tablet window, with 200 mg of ginger and 50 mg of peppermint to support digestive comfort and 250 mg of magnesium to support regular bowel movements. Ginger tea after dinner does part of the same job.*
  6. Keep a short log. Dose time, nausea from 0 to 10, meals and bowel movements for the 30 days on 1.5 mg. It shows your prescriber how the start went before the step to 4 mg.
  7. Tell your prescriber about your other pills. The label says the tablet can change how some oral medicines are absorbed; in one study levothyroxine levels rose 33 percent. Ask about thyroid pills, blood thinners and other closely monitored medicines.

Two kinds

Normal for week 1, or a same-day call

Usually normal

  • Mild nausea in the hours after the dose
  • Full after a few bites
  • Less appetite
  • Burping, a little bloating
  • A mild headache or tiredness

Call the same day

  • Vomiting more than once, or no liquids staying down
  • Severe belly pain, especially into the back
  • Swelling of face, lips or throat
  • Pain under the right ribs, fever, yellow eyes
  • Shaky, sweaty spells on insulin or a sulfonylurea

Week 1 should feel like a nudge, not a knockdown.

When to call your doctor

Call your prescriber the same day if:

  • You vomit more than once in a day or cannot keep liquids down
  • You have severe belly pain that will not go away, especially if it spreads to your back
  • You have swelling of the face, lips, tongue or throat, or trouble breathing (stop the tablet and get emergency help)
  • You have pain under the right ribs, fever, or yellow skin or eyes
  • You feel faint or pass much less urine
  • You take insulin or a sulfonylurea and get shaking, sweating or confusion

Mild and fading is week 1. Severe or building is a call.

Questions people also ask

How long do Wegovy pill side effects last in the first week?

Usually a few hours a day, mostly after the dose, easing by afternoon. The level is still building through the first month, and the next bump usually comes with the step to 4 mg on day 31. If symptoms grow day by day in week 1, or you cannot eat or drink, call your prescriber rather than waiting.

Why do I feel sick right after taking the Wegovy pill?

Because it goes into an empty stomach and is absorbed there, peaking about an hour after the dose, per the label. Lying back down, a large breakfast or coffee as breakfast all make it worse. Sit upright, wait the full 30 minutes, then eat something small and plain with a little protein.

Is the Wegovy pill easier on the stomach than the shot?

Not clearly. The label says side effects with the 25 mg tablet were similar in type and frequency to the injection. The pattern differs: a daily tablet gives a flatter level than a weekly shot, so instead of a wave after injection day, some people get a short queasy stretch after each dose.

Can I take the Wegovy pill at night instead if mornings are bad?

Not on your own. The label says to take it in the morning on an empty stomach, and absorption depends on that fasting window. If mornings are hard, change what comes after: the full 30 minutes, a small protein breakfast, coffee after food. If nausea stays strong, ask your prescriber about staying longer on a dose.

When should I worry about constipation on the Wegovy pill?

A slower week is common once appetite drops. Water, oats or other fiber, and a short walk after dinner usually help. Call if you go more than five days without a bowel movement, your belly is swollen and hard, or you are vomiting. See whether constipation is normal on a GLP-1.
