Why does hair fall out on a GLP-1 (telogen effluvium)?

The short answer

Rapid weight loss and a sudden drop in what you eat push more hairs than usual into their resting phase. Those hairs let go two to four months later, the shedding lasts a few months, and new growth follows. This is telogen effluvium. It is listed in the Wegovy and Zepbound prescribing information, and it is temporary.

What is actually happening

Every hair on your head runs its own cycle: a growing phase measured in years, a resting phase of a few months, then it falls out and the follicle starts a new hair. On a healthy scalp about 85 percent of hairs are growing and 15 percent are resting at any moment, which is why losing 50 to 100 hairs a day is normal. Telogen effluvium is what happens when a shock to the body pushes a much larger share of follicles into rest at once.

On a GLP-1 the shock is not the medication acting on the follicle. It is what the medication makes possible: fast weight loss and a large, sudden drop in calories, protein and minerals. Hair is not essential tissue, so when supply falls the body quietly stops investing in it. Follicles that were mid-growth switch to resting. Nothing is visible yet.

The delay is what confuses people. A resting hair stays anchored for roughly three months before it lets go, so the shedding starts two to four months after the trigger, often when the weight loss is already slowing. It shows up as more hair in the brush and the drain, and as thinning across the whole scalp rather than a bald patch. Acute telogen effluvium lasts less than six months by definition, usually much less, and because the follicles are intact, new hair is already growing in as the old hair falls.

The Wegovy prescribing information lists hair loss as an adverse reaction reported more often than with placebo. The Zepbound label goes further: it states that hair loss was associated with weight reduction and was reported far more often in women than in men. The makers describe it as a consequence of losing weight, not a direct drug effect. Eating far less also means less protein, iron and zinc, and low protein intake and iron deficiency are themselves recognized triggers of telogen effluvium.

Thinning all over, months after the trigger, is telogen effluvium. Patches are something else.

What to do

  1. Set a daily protein floor. Hair is built from protein, and on a GLP-1 appetite drops faster than need. Ask your prescriber or a dietitian for a daily gram target and put protein first at every small meal.
  2. Get iron checked, not guessed. Ask your prescriber for a ferritin and iron panel, since iron deficiency is a recognized trigger of shedding and is common in women eating far less. Do not start iron on your own; too much has its own problems.
  3. Cover zinc, biotin and B vitamins. Eggs, meat, fish, beans, nuts and dairy carry most of them. If your meals are now very small, a daily formula built for this phase is a reasonable backstop.
  4. Slow the drop if you can. Losing weight very quickly is the trigger. If your intake has collapsed below what feels livable, tell your prescriber; staying longer on the current dose is a normal request.
  5. Be gentle with what you have. Skip tight ponytails, heavy heat and bleaching for a few months. None of these cause telogen effluvium, but they snap hairs that are already thinning.
  6. Count in months, not days. Write down the date the shedding started. Expect it to ease within a few months and fullness to return over six to nine, which is the timeline the American Academy of Dermatology describes.
  7. Watch the pattern, not the total. Diffuse thinning across the scalp is telogen effluvium. Patches, a receding line, or shedding that continues past six months is something else and belongs with a dermatologist.

  • You feel faint when you stand
  • You are keeping down almost no food or liquid
  • The shedding comes with a racing heart or breathlessness on stairs
  • You feel cold all the time
  • Hair is coming out in patches with bare skin showing
  • Your scalp is red, scaly or painful
  • Eyebrows or body hair are thinning too
  • The shedding has gone on for more than six months

Does Ozempic cause hair loss?

Not directly. The follicle is not the target of the medication. What causes the shedding is the rapid weight loss and the sharp fall in calories, protein and minerals that the medication makes possible. The Wegovy label, which is the same molecule at a higher dose, lists hair loss as reported more often than with placebo, and the Zepbound label says plainly that it was associated with weight reduction.

How long does hair loss last on a GLP-1?

Usually a few months. The shedding starts two to four months after the trigger, peaks, and by definition acute telogen effluvium lasts less than six months, often much less. Regrowth is already under way while the old hair falls. The American Academy of Dermatology describes fullness returning over six to nine months once the shedding stops.

Will my hair grow back after Wegovy or Zepbound?

In telogen effluvium, yes. The follicles are not damaged; they paused. Once weight loss slows and protein, iron and zinc are back to adequate levels, follicles return to their growing phase and new hairs come in as short, fine regrowth along the hairline and part. If the thinning is patchy or continues past six months, see a dermatologist, because that is a different problem.

What vitamins help with hair loss on a GLP-1?

Only the ones you are short of. Protein comes first, then iron, but iron should be tested before it is taken. Zinc, biotin and B vitamins support normal hair and are easy to run low on when meals shrink.
