How to taper off a GLP-1 without regaining

The short answer

No GLP-1 label includes a tapering schedule, and no trial has shown that stepping down slowly stops regain. The trials do show three things: stopping brings back most of the weight within a year, staying on a lower dose keeps more of it off than stopping, and exercise helps hold the loss after treatment ends. Plan any step-down with your prescriber.

At GLP-1 SOS we hear this most from people who have reached a weight they are proud of and want to step off without losing it. The honest answer starts with what is not known. If you have already stopped and feel off, read why you may feel not normal months after quitting. This page is about the plan before the last dose.

At a glance

Three things the evidence says

1

No proven taper

No label has a taper schedule, and the big trials stopped people all at once. A slow step-down is reasonable, but it is not a tested shield against regain.

2

Stopping brings weight back

A year after stopping semaglutide, STEP 1 participants had regained about two thirds of what they lost.

3

Lower beats zero

In SURMOUNT-MAINTAIN, people who dropped to 5 mg of tirzepatide kept more weight off than people switched to placebo.

What is actually happening

A GLP-1 lowers appetite only while it is in your body. Semaglutide has a half-life of about one week and stays in the circulation for about 5 to 7 weeks after the last dose, per the Wegovy label; tirzepatide’s half-life is about 5 days. As the level falls, fullness arrives later, food noise returns, and a body that lost weight pushes back with stronger hunger. That is physiology, not a lack of willpower.

The trials that tested stopping were blunt. In the Zepbound withdrawal study, people lost 20.9 percent over 36 weeks; those then switched to placebo regained 14 percent of their body weight over the next year, while those who stayed on kept losing. In the Wegovy withdrawal study, people switched to placebo after 20 weeks gained 6.9 percent, while those who continued lost another 7.9 percent.

Lab note

A taper changes how fast the medication leaves. It does not change what the medication was holding back.

The closest thing to taper evidence is a dose-reduction trial. In SURMOUNT-MAINTAIN, adults who had lost weight on full-dose tirzepatide were split three ways for a year. Measured from the start, those on the full dose ended at minus 21.9 percent, those who dropped to 5 mg at minus 16.6 percent, and those on placebo at minus 9.9 percent. Stepping down kept more weight off than stopping, but less than staying on.

One more signal points to what you control. In a Copenhagen trial with liraglutide, people who had done a year of supervised exercise held on to more of their loss a year after treatment ended than people who had only taken the medication.

What to do

  1. Decide the goal with your prescriber first. Stopping, staying on a lower dose, or spacing doses are different plans. Only a lower maintenance dose has trial data behind it so far, and any dose change is a prescriber decision.
  2. If you step down, go one dose at a time. Ask your prescriber to hold each lower dose for several weeks before the next change. There is no tested schedule, so the pace should follow your hunger and your weight.
  3. Weigh weekly and write one hunger note. Same day, same time, same scale. Agree with your prescriber on a number that triggers a call.
  4. Build the habits while the medication still helps. Keep the small plate, protein first, and set meal times rather than grazing. A daily target from how much protein to eat on a GLP-1 is a good anchor.
  5. Add strength work now, not after. Two strength sessions a week plus regular walking. Exercise held weight best after treatment ended in the Copenhagen trial, and it protects the muscle we explain in whether a GLP-1 causes muscle loss.
  6. Keep the gut routine steady while doses change. As the dose falls, the gut often speeds up after months of slow. Halve a laxative as stool loosens instead of quitting overnight.
  7. Have a plan B written down. Know what you will do if regain passes your number: call about a lower maintenance dose or restarting. After a gap, restarting usually means lower doses first; the Wegovy label says to restart escalation at a lower dose if 2 or more weekly doses are missed.

From our support desk at GLP-1 SOS

If you wrote to us tonight about this, here is what we would say.

Nothing we make, and nothing on any shelf, stops regain after a GLP-1. Plan the step-down with your prescriber, weigh weekly, protect your muscle, and keep a written plan B. Where we can help is the gut, which often swings for a few weeks as the dose falls. We built GLP-1 SOS Daily Gut Relief for a steady evening routine: 250 mg of magnesium to support regular bowel movements, plus ginger and a 19-strain probiotic blend for digestive comfort, in 2 capsules with dinner. Magnesium citrate and ginger tea are the brand-free version.*

We answer real questions from real people every day. Write to us if yours is not here.

Two kinds

Expected during a step-down, or a call

Usually expected

  • Hunger arriving earlier in the day
  • More thoughts about food
  • Larger portions feeling comfortable again
  • A few pounds of drift in the first weeks
  • Bowels speeding up after months of slow

Call your prescriber

  • Weight climbing several pounds a week
  • Eating that feels out of control
  • Blood sugar above your targets if you have type 2 diabetes
  • Severe vomiting after restarting a dose
  • Low mood that does not lift

Some regain is the physiology of stopping. Racing regain is a reason to revisit the plan, not a failure.

When to call your doctor

Call your prescriber if, during a step-down or after stopping:

  • Your weight climbs several pounds a week, or passes the number you agreed on
  • You take Ozempic or Mounjaro for type 2 diabetes and your blood sugar runs above your targets
  • You restarted at your old dose after a gap and cannot keep liquids down
  • Eating feels out of control
  • Your mood stays low. If you have thoughts of harming yourself, call or text 988 (Suicide & Crisis Lifeline)
  • Severe belly pain that may spread to your back

A taper is a plan you make with your prescriber, not a schedule you guess at.

Questions people also ask

Is there a proven way to taper off Ozempic or Wegovy?

No. The Ozempic and Wegovy labels have no tapering schedule, and the large trials that tested stopping stopped people all at once. A gradual step-down with your prescriber is reasonable, mainly to watch how hunger and weight respond at each dose. What has trial evidence is staying on treatment, or on a lower dose, which kept more weight off than stopping.

Will I regain all the weight if I stop Zepbound or Mounjaro?

Not necessarily all, but most people regain a large share. In the Zepbound withdrawal study, people switched to placebo regained 14 percent of their body weight over a year after losing about 21 percent. Regain varies widely. Strength work, a protein target and weekly weigh-ins help you catch it early.

Can I stay on a lower dose of tirzepatide instead of stopping?

That is the only step-down option tested in a trial. In SURMOUNT-MAINTAIN, people who dropped to 5 mg after losing weight on the full dose ended at minus 16.6 percent from where they started, versus minus 21.9 percent on the full dose and minus 9.9 percent on placebo. Whether a lower dose fits you is your prescriber’s call.

Should I space out my shots to every two weeks to wean off?

Not on your own. No GLP-1 label describes every-other-week dosing, and it has not been tested as a way to avoid regain. Gaps also matter: the Wegovy label says to restart at a lower dose after 2 or more missed weekly doses, to limit stomach side effects. If you want to stretch doses, ask your prescriber what fits your medication.

How long does it take for hunger to come back after stopping a GLP-1?

It follows how fast the medication leaves. Semaglutide stays in the circulation for about 5 to 7 weeks after the last dose, and tirzepatide clears sooner, so fullness fades within weeks and hunger rises after that. If you feel off months later, read why you may not feel normal after quitting.
