Definition
Orforglipron (Foundayo) is a once-daily GLP-1 receptor agonist pill. It is a small molecule, not a peptide, and it can be taken with or without food. It was first approved in the US in 2026 for weight reduction and long-term weight maintenance in adults.
Most GLP-1 medicines are peptides, chains of amino acids. Orforglipron is a nonpeptide molecule that still activates the human GLP-1 receptor. Unlike oral semaglutide, it has no empty-stomach rule: the Foundayo label says to take it with or without food and reports no clinically relevant food effect, a half-life of about 29 to 49 hours and steady levels after about one week. It is processed by the liver enzyme CYP3A4, so some medicines change its dose limits: the label caps Foundayo at 9 mg a day with strong CYP3A4 inhibitors and limits simvastatin to 20 mg a day.
The label starts at 0.8 mg once a day, then 2.5 mg and 5.5 mg after at least 30 days on each dose, with optional steps to 9, 14.5 and 17.2 mg based on response and tolerability. Side effects mirror the class: nausea in 26% to 35% vs 10% on placebo, and constipation in 20% to 27% vs 9%, mostly during dose increases. If you use birth control pills, the label advises a non-oral method or a barrier method for 30 days after starting and after each increase.
In the 72-week ATTAIN-1 trial, the highest dose tested lowered body weight by an average of 11.2% vs 2.1% on placebo. Compare it with the other pill, oral semaglutide. At GLP-1 SOS we expect more of our community to start on a pill, and the gut side effects are handled the same way.
