Oral semaglutide (Rybelsus, Wegovy tablets): what it means on a GLP-1

Definition

Oral semaglutide is semaglutide in a once-daily tablet: Rybelsus and Ozempic tablets for type 2 diabetes, and Wegovy tablets for weight and heart risk. It is the same molecule as the weekly shot, formulated to be absorbed from the stomach.

The tablets contain an absorption enhancer called SNAC that helps semaglutide get absorbed, mostly in the stomach. Even so, only about 0.4% to 2% of the dose reaches the blood, depending on the tablet. That is why tablet doses are much higher than shot doses: the Wegovy label predicts that a 25 mg daily tablet gives blood levels comparable to the 2.4 mg weekly shot, with more variation from person to person.

That small absorption window is why the rules are strict. The labels say to take the tablet once a day on an empty stomach in the morning with up to 4 ounces of plain water, swallow it whole, and wait at least 30 minutes before eating, drinking anything else or taking other pills. Wegovy tablets start at 1.5 mg a day, then go to 4 mg, 9 mg and the 25 mg maintenance dose, every 30 days. If you miss a day, the labels say to skip it and take the next dose the following day.

In the OASIS 4 trial, adults on 25 mg lost an average of 13.6% of body weight over 64 weeks vs 2.2% on placebo, and stomach and bowel side effects were reported by 74.0% vs 42.2%. The other GLP-1 pill works differently; see orforglipron (Foundayo). At GLP-1 SOS we often hear that the 30-minute wait is the hardest part, especially for coffee lovers.
