Missed a GLP-1 dose: what to do

The short answer

It depends on the medication, and each label spells it out. Ozempic: take the missed shot within 5 days, otherwise skip it. Wegovy shot: take it if your next dose is more than 2 days away, otherwise skip it. Zepbound and Mounjaro: take it within 4 days (96 hours), otherwise skip it. Rybelsus, Ozempic tablets and Wegovy tablets: skip the missed pill and take the next one the following day. Never take two doses to catch up. After two or more missed weeks, call your prescriber before restarting.

At GLP-1 SOS we get this message most often on a Monday: “I forgot my shot on Saturday. Do I take it now, wait, or double up next week?” The good news is that the brand-name labels answer it precisely, and the answer is rarely “do nothing” and never “double up.” The rules below are copied from each Prescribing Information. Find your medication, count the days, and you will know what to do in under a minute.

At a glance

Three rules that cover every label

1

Count from the missed day

Weekly shots have a window: 5 days for Ozempic, 4 days for Zepbound and Mounjaro, and for Wegovy, more than 2 days before the next shot.

2

Outside the window, skip

Then take your next dose on your regular day. Daily semaglutide pills are simply skipped.

3

Never double

No label allows two doses to make up for one. Several missed weeks may mean restarting lower.

What each label says, word for word

Ozempic: “If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day.” Wegovy injection: if one dose is missed and the next scheduled dose is “more than 2 days away,” take it as soon as possible; if it is “less than 2 days away,” do not take it and resume on your regular day. In a seven-day week, that works out to roughly the same window as Ozempic: up to about 5 days late.

Zepbound and Mounjaro: take the missed dose “as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day.” The difference from semaglutide is not arbitrary: both tirzepatide labels say doses must be at least 3 days apart, while semaglutide needs at least 2. If you take a late shot inside the window, you can then go back to your usual weekly day, as the labels say.

Lab note

Late inside the window: take it. Outside the window: skip it. Never: two at once.

Daily pills are simpler. The Rybelsus and Ozempic tablets label and the Wegovy tablets label say the same thing: skip the missed dose and take the next one the following day. Foundayo is different: take the missed dose as soon as possible, but do not double up the next one. Saxenda, a daily shot, says resume with the next scheduled dose and do not take extra. Compounded semaglutide and tirzepatide have no FDA-approved label, so ask the prescribing clinic or pharmacy for their rule. If the gap turns into weeks, read how long a GLP-1 stays in your system.

From the labels

The numbers to remember

5 days

the window to take a missed Ozempic shot; the Wegovy rule (next dose more than 2 days away) lands in about the same place

4 days

the window for Zepbound and Mounjaro (96 hours), per both tirzepatide labels

2 in a row

missed Wegovy shots before the label says to restart at a lower dose; Foundayo says 7 missed days, Saxenda more than 3

What to do

  1. Count the days since your missed dose. Use the day you should have taken it as day zero. For Wegovy, count instead how many days remain until your next scheduled shot.
  2. Inside the window, take it now. Up to 5 days late for Ozempic, more than 2 days before the next Wegovy shot, or up to 4 days late for Zepbound and Mounjaro. Then go back to your usual day the following week.
  3. Outside the window, skip it. Do not take it. Take the next dose on your regular day. Do not move your whole schedule to the late day unless your prescriber tells you to; our page on changing your shot day explains the spacing rules.
  4. Never take two doses to catch up. If you already did, call your prescriber or the Poison Help Line at 1-800-222-1222, and watch for severe nausea, vomiting, or signs of low blood sugar if you also take insulin or a sulfonylurea.
  5. Missed two or more weeks? Call before restarting. The Wegovy label says to restart at a lower dose after two missed doses in a row to reduce stomach side effects. The other weekly labels give no restart schedule, so let your prescriber choose the dose.
  6. Expect the first week back to feel like week one. Nausea and constipation can return. Keep meals small and plain, sip water, and keep a steady evening routine: 2 capsules of GLP-1 SOS Daily Gut Relief with dinner, with magnesium that supports regular bowel movements and ginger and peppermint that support digestive comfort.*
  7. Make the next miss less likely. Set a repeating phone alarm for shot day, keep pens where you will see them, and plan ahead for travel. Our injection day tips help build the habit.

Two kinds

Handle it yourself, or call first

Usually fine to follow the label

  • One weekly dose, still inside its window
  • One weekly dose, past the window: skip it
  • One missed daily semaglutide pill: skip it
  • Back on your usual day the next week

Call your prescriber first

  • Two or more weekly doses missed in a row
  • You took two doses close together
  • A compounded dose and no written rule
  • You take insulin or a sulfonylurea and are unsure

When in doubt, your pharmacist can read the rule from your label with you.

When to call your doctor

Call your prescriber the same day if:

  • You took two doses close together, or more than your prescribed dose
  • You missed two or more weekly doses in a row and are about to restart
  • You have severe stomach pain that will not go away, sometimes spreading to your back
  • You are vomiting and cannot keep fluids down, or feel dizzy and are making little urine
  • You have shakiness, sweating, confusion or a fast heartbeat and take insulin or a sulfonylurea
  • You have swelling of the face, lips or throat, or trouble breathing

One late dose is a schedule problem. Two doses at once is a call.

Questions people also ask

I missed my Ozempic shot. Can I take it 3 days late?

Yes. The Ozempic label says to take a missed dose as soon as possible within 5 days after the missed dose, so 3 days late is inside the window. Then take your next dose on your regularly scheduled day. If more than 5 days have passed, skip the missed dose and wait for your usual day. Do not take two doses to make up for it.

What if I missed my Wegovy shot by 6 days?

Skip it. The Wegovy label says to take a missed shot only if the next scheduled dose is more than 2 days away. Six days late means your next shot is about a day away, so you resume on your regular day instead. If you missed two or more doses in a row, the label says to restart at a lower dose; call your prescriber before your next shot.

How late can I take Zepbound or Mounjaro?

Up to 4 days (96 hours) after the missed dose, according to both tirzepatide labels. After that, skip the missed dose and take the next one on your regular day. Either way, you can then go back to your usual weekly schedule. The labels also say two tirzepatide doses must be at least 3 days apart, which is why the window is shorter than for semaglutide.

I forgot my Rybelsus or Wegovy pill. Should I take it later in the day?

No. The Rybelsus and Ozempic tablets label and the Wegovy tablets label both say to skip the missed dose and take the next dose the following day. These pills need an empty stomach and a 30-minute wait before food, so a later dose would not follow the rules anyway. Foundayo is the exception: take it as soon as you remember, but never two in one day.

I missed two weeks of my GLP-1. Can I restart at my usual dose?

Ask your prescriber first. The Wegovy label says that after two or more missed doses in a row, you should restart dose escalation at a lower dose to reduce stomach side effects. Ozempic, Zepbound and Mounjaro labels do not give a restart schedule, so your prescriber decides.
